[Abstract]
ObjectivesTo Provid scientific evidence for cancer prevention, the epidemic trends of cancer was studied with cros-sectional study.Subjects and methodsThe new cases and deaths from cancer in Tanghai from 1995 to 2006 were coded according to ICD-9.Clinical information(the first sourse) of malignant tumors was collected from the 5 biggest hospitals (The affiliated hospital of my university,Kailuan hospital,Women and children?hospital,Workers?hospital,Hospital for people ) in Tangshan downtown and the biggest hospital of Tanghai County. Death records were collected from police substations in Tanghai County and selected those died of prevalent malignant diseases as the second source. The incidence rate and tendency of malignant tumors were evaluated with Capture-recapture Method.The incidence of cancer was calculated by groups of calendar year, genders and ages, furthermore, the secular trends of cancer during the decade were analyzed. Relative risks for cancer incidence were estimated for people in different generations, different genders and ages. Cancers of different sites were ranked by various incidence indexes to show the relative seriousness from assorted respects. All the data was handled by Excel and SPSS 11.5.ResultsTotally 998 new cases (male: 526, female: 472) and 668 deaths (male: 473, female: 195) were collected in Tanghai from 1995 to 2006. 211 persons were captured by the two materials, and the total estimated number was 3066.The top 10 cancer sites by estimated incidence rate were: lung cancer, liver cancer, stomach cancer, breast cancer, colon cancer, leukemia, brain tumors, esophageal cancer, bladder cancer and lymphoma. For males, the top 5 cancer sites by estimated incidence rate were: lung cancer, liver cancer, stomach cancer, esophageal cancer and colon cancer. And the top 5 cancer for females were: lung cancer, breast cancer, stomach cancer, liver cancer and colon cancer.In the last decade, the crude incidence rate for overall cancers increased year by year, so did the estimated and the age standardized incidence rate. Statistical increasing trends for age standardized incidence rate were found in 6 of the top 10 cancer sites which were lung cancer, liver cancer, breast cancer, leukemia, brain cancer and lymphoma. During the last decade, cancer incidence rate increased obviously at age group 0, age group 75 and age group 45. The peak age specific rate of cancer incidence was observed at age group 65 and age group 75. The peak incidence rate of breast cancer was at age group 55 to 65 while it was at age group 65 to 75 for the other 4 cancers in top 5 cancer sites.The mean Age of Onset for liver cancer was lower for male than that for female, while the mean Age of Onset was higher for male than that for female for stomach cancer and leukemia. The mean Age of Onset for leukemia was lower than that for other cancers in top 10 sites except brain tumors, and the mean Age of Onset for lung cancer, stomach cancer, esophageal cancer were higher than that for breast cancer and brain tumors, furthermore, the mean Age of Onset for esophageal cancer was higher than that for liver cancer. The proportion of male and female cases fluctuated during the decade. The risk by sex was changed and decreased from 2001 then.There were more male patients than female patients for lung cancer, liver cancer, stomach cancer, leukemia, brain tumors, esophageal cancer, bladder cancer and lymphoma, while it was reversed in breast cancer and colon cancer.ConclusionsThe ascending trends of cancer in Tanghai attribute largely to the ageing population. The aged is by all means the high risk group of cancer occurrence, therefore, the health-care of the old should be reinforced to improve the prevention, diagnosis and treatment of cancer, and to enhance survival. Lung cancer occupied the top of all cancers with its overwhelming incidence; it threatens more and more severely the health of the public because of its dramatic increase. By the result,the prevention of lung cancer should be the focus of cancer control in the future.There were 3 kinds of enteron cancer in top 5 cancer sites,therefore, we should still pay attention to enteron cancer in cancer control.The Age of Onset of breast cancer was earlier than the that for other cancers in top 5 cancer sites.
